We are currently hiring Licensed Clinical Social Workers who are compassionate and looking to make a difference in the lives of seniors. Training and ongoing support are provided by our clinical and administrative teams. We are dedicated to helping our clients improve their emotional, cognitive, and daily functioning, along with adjustment to their current life circumstances. Our clinicians are hired as Independent Contractors.

Requirements include:  Master’s Degree in Social Work and an active Clinical Social Worker license in Illinois

Positions available in:  Hoffman Estates, Palatine, Northbrook, Elmhurst, Lombard. and Chicago. 

Responsibilities include, but are not limited to:

• Conduct initial assessments of clients’ psychological, emotional, cognitive, and social functioning.
• Complete a treatment plan for each client that includes goals, objectives, and planned services.
• Conduct psychotherapy and cognitive training sessions up to twice weekly.
• Provide ongoing communication with clients, families, community staff and healthcare providers.
• Enter and maintain all documentation in the electronic health record within required timeframes.
• Attend required training or continuing education as needed.
